Raw registered patients (PDS headcount) — not the weighted/adjusted list used for QOF/IIF payments. Month-to-month moves reflect a mix of real population change, list cleansing/validation, and practice closures & mergers (which can show as very large jumps). Practices below 750 patients at the start of the window are excluded from change stats to limit merger noise.
