What it does

• One record per system — supplier assurance, local assessment, hazard log, safety-case documents, the decision to deploy, and everything that happens afterwards.
• A 15-item local assessment across five stages, eight of them mandatory, each with a plain-English description of what "done" means.
• A proper hazard log on the NHS five-by-five matrix, scored before and after controls, with residual risks that need explicit acceptance flagged rather than buried.
• Draft documents generated from your record — Clinical Risk Management Plan, hazard log, safety case report and a staff briefing, as editable Word files for your Clinical Safety Officer to finish and sign.
• Incidents, actions and periodic reviews that keep the record alive after go-live, with an append-only audit trail behind all of it.

What it does not do

It does not make clinical safety judgements for you, and it will not tell you a deployment is safe. It records and organises the work your practice does, and every document it produces is a draft requiring review and sign-off by the person holding clinical safety oversight. The responsibility stays with your organisation — this makes it possible to discharge it and show that you did.

Restricted to practice staff

The register is scoped to your practice workspace and visible only to active members of that practice. A PCN workspace shows nothing — a PCN does not hold its member practices' DCB0160 duties, and one practice's safety record has no business in front of another practice's staff.
